A 2 year old child with Diamond Blackfan anaemia (DBA) is to be started on a dose of 2mg/kg of prednisolone for an exacerbation of anaemia. Which ONE of the following strategies (a-e) would you consider to be LEAST appropriate

a) Bisphosphonates to prevent secondary osteoporosis

b) Blood transfusion

c) Hepatitis B immunisation

d) Trimethoprim-sulphamethoxazole for Pneumocystis jiroveci prophylaxis

e) Varicella immunisation

Answer:

a) Bisphosphonates to prevent secondary osteoporosis

Explanation:

Bisphosphonates are not currently recommended for prevention of secondary osteoporosis in DBA patients on long term or high dose steroids although avascular necrosis of the femoral head remains a complication. Varicella and hepatitis B immunisation are recommended prior to commencing therapy and Pneumocystis prophylaxis for prolonged or high dose steroid courses.
