A 48 year old man presents with retinal haemorrhages and a plasma viscosity of 4.5 cp. Which ONE of the following drugs should be included as part of multiagent first line treatment following emergency plasmapheresis?

a) Bortezomib

b) Chlorambucil

c) Doxorubicin

d) Fludarabine

e) Melphalan

Answer

a) Bortezomib

Explanation:

The preferred first line treatment regime for rapid disease control should contain bortezomib and dexamethasone. This has shown a response rate (measured by IgM reduction >25%) of 1.4 months compared to 3-4 months with other combinations. Purine analogues and continuous oral alkylating agents should be avoided if a patient is a potential candidate for autologous stem cell transplantation.
